What each combination actually produces
Bleach and ammonia release chloramine vapors. Symptoms start as burning eyes, coughing, and chest tightness, and heavier exposure causes real airway injury. Ammonia is in many glass cleaners and in some multi-surface sprays, and it is also present in urine, which is how bathroom incidents happen.
Bleach and an acid release chlorine gas. Toilet bowl cleaners, descalers, rust removers, and vinegar are all acidic. This is the fastest-acting of the common combinations.
Bleach and rubbing alcohol produce chloroform and related compounds. That combination has become more common because alcohol-based products are widely kept in kitchens and bathrooms.
Peroxide and vinegar mixed and stored in one bottle form peracetic acid, which is corrosive to skin, eyes, and airways. Used one after the other with a rinse between, they are fine; combined in a container, they are not.
Two different drain cleaners together can react violently and eject hot caustic liquid out of a drain.
Why this happens by accident, not by ignorance
Very few people deliberately mix products. What happens is sequential: a bathroom cleaner goes on, it does not work well enough, and a second product goes on top of the first while it is still wet.
Drains are the other common path. Two products poured into the same sink or toilet meet in the trap, out of sight, and the gas comes back up.
The third path is a mop bucket or spray bottle refilled with a different product without being washed out. Residue from the previous product is enough.

The rule that prevents all of it
One product at a time. Apply it, let it work, rinse the surface thoroughly with clean water, and only then consider a different product.
Never pour two products down the same drain, even minutes apart. Flush with plenty of water in between.
Never store a mixed solution. Homemade blends kept in a bottle can react slowly, build pressure, or degrade into something more hazardous than the ingredients.
Ventilate before starting, not after something goes wrong. Open a window and run the exhaust fan, and never work with strong products in a closed bathroom.
What to do if you have already mixed something
Leave the area immediately and take everyone and every pet with you. Do not stay to open windows first, and do not try to dilute or neutralize it.
Get to fresh air, then ventilate from outside the space if you can do it safely, by opening a door and leaving again.

Anyone with breathing symptoms needs medical assessment, and that is a doctor's decision rather than something to wait out. Birds are extremely sensitive to fumes and should be moved out of the house entirely, and pets belong outside the area as well.
Ventilation is a step, not a precaution
Household cleaning fumes accumulate fastest in exactly the rooms people use the strongest products in: small bathrooms and closed laundry areas.
Run the exhaust fan for the whole job and for a while afterward, and open a window even in cold weather. If the smell is strong enough to notice, the ventilation is not adequate.
